  • The prevalence of skin infections linked to tattoos is estimated at around 1 to 5%.

Why Red Ink Causes the Most Reactions

Ask any experienced tattoo artist in Melbourne, and they’ll tell you: red is the one colour clients are most likely to have trouble with. There’s a solid scientific reason for that.

  • Mercury, Cadmium and Modern Pigment Substitutes

Research analysing red tattoo pigment from 18 patients who developed cutaneous reactions found aluminium, iron, calcium, titanium, silicon, mercury and cadmium within the pigment, all of which can trigger cutaneous inflammation. 

Red inks containing mercury, cadmium, or azo compounds can trigger delayed type IV allergy. This is a hypersensitivity reaction that involves the immune system ramping up days, weeks, or even months after exposure, which is why red tattoo reactions are so often mistaken for something else entirely.

While many modern tattoo inks have moved away from mercury and cadmium due to their association with skin reactions, allergic reactions to red pigments still occur. 

Beyond red ink, other colours carry their own risks worth knowing about:

Natural, pure henna is generally safe. The problem is that most “black henna” is not pure henna at all. To make temporary henna tattoos look more like a real tattoo, para-phenylenediamine (PPD) is added to create what’s sold as “black henna.” No natural black henna actually exists. 

PPD is a chemical used in hair dyes and textile dyes, and it is a potent allergen.

  • Studies have found PPD present in black henna tattoo mixtures at concentrations as high as 15.7%, significantly higher than levels found in commercial hair dye preparations. 
  • The allergic contact dermatitis reaction from PPD typically appears 7 to 14 days after first exposure in someone who hasn’t been sensitised before. In someone already sensitised to PPD, the reaction can develop within 48 hours. 
  • PPD sensitisation from a henna tattoo can cause lifelong reactivity to hair dyes, rubber chemicals, clothing dyes, and some medications, including sulfonamides and hydrochlorothiazides. 

This is not a minor inconvenience. In serious cases, reactions to PPD can cause blistering, scarring, and systemic responses, including urticaria and anaphylaxis.

  • Always check what is in any “henna” product before it’s applied

Late-Onset Allergies: Reactions That Appear Years Later

If your tattoo has been sitting peacefully on your skin for years and suddenly starts to react, it does not mean you got a bad tattoo.

What Triggers a Sudden Reaction to Old Tattoos?

Several things can cause the immune system to suddenly respond to pigment that has been sitting in your skin for a long time:

  • A new illness or infection that shifts your immune response
  • Pregnancy or hormonal changes that alter immune tolerance
  • Medications that affect immune function
  • Sun exposure (more on this below)
  • Stress, which can trigger or worsen many skin conditions

It is possible to develop an allergy to a tattoo as you age and experience adverse effects years after getting the tattoo. 

Reactions After Joint Replacement or Medical Treatment

This is one of the more unusual but well-documented triggers. Some people develop an allergic reaction to their tattoo after undergoing a medical treatment, and this is most likely to occur following joint replacement surgery. 

Joint replacement hardware releases trace amounts of metal ions into the bloodstream. If your tattoo ink contains similar metals (nickel and cobalt are common to both), the immune system can become cross-sensitised, suddenly “seeing” the pigment in your tattooed skin as a threat.

In rare cases, tattoos can also cause swelling or burning during an MRI scan and may interfere with imaging quality. This is worth disclosing to your medical team if you’re scheduled for a scan.

Tattoos and Pre-Existing Skin Conditions

 

  • Eczema, Psoriasis and the Koebner Phenomenon

If you have an existing skin disease, getting a tattoo requires some extra thought. People with eczema, psoriasis, or lichen planus need to be particularly careful.

Photosensitivity is a lesser-known but genuinely frustrating tattoo complication. Some tattoo pigments, particularly yellow and red ones, can cause a reaction when exposed to UV light.

What this looks like:

  • Raised, itchy skin over coloured ink areas after sun exposure
  • Redness and swelling that appear specifically over the tattooed skin (not surrounding areas)
  • Symptoms that resolve when you’re out of the sun, then return next time you’re exposed

This is classified as photodermatitis, and it’s a form of skin reaction triggered by the interaction between UV radiation and certain tattoo pigments. Azo pigments in tattoo ink can form harmful compounds inside the skin under the influence of UV radiation. 

Protecting tattooed skin from direct sun exposure is a good practice for everyone, not just those with photosensitivity. It preserves the ink and reduces your risk of a reaction. Use SPF 50+ sunscreen and reapply regularly, especially during Australian summers.

When to See a Dermatologist (and What They Will Do)

Not every tattoo reaction needs an emergency visit. But some do. Here’s a guide:

See a doctor urgently if you have:

  • Difficulty breathing or throat tightness after a tattoo session
  • Rapid spreading of redness, warmth and swelling (possible infection)
  • High fever alongside skin changes at the tattoo site
